It is the policy of Madras Physical Therapy to treat all of our patients equitably and with dignity in accordance with the Financial Hardship Policy. Discounts offered to patients are intended for residents of the community served by Madras Physical Therapy. Our practice reserves the right to modify our Patient Discount Policy at any time.

Financial Hardship Application Process

1.  Financial Hardship Application Process:  To ensure that decisions to provide patients with financial assistance are documented and based on upon uniform and objective criteria, each patient who desires a discount based on financial hardship must complete the attached confidential Financial Hardship Worksheet (click here to open and print to complete).

In addition to information provided on the Financial Worksheet patients must provide proof of their household income, such as a copy of their most recent federal tax return or payment stubs.  If unable to provide documentation of income, patients should discuss these challenges with our clinical staff to consider alternatives.

Madras Physical Therapy will re-verify eligibility for Financial Hardship Discounts every 6 months.

2.  Federal Law.  In accordance with federal law, discounts offered under this policy are not intended and shall not operate to generate business payable under the Federal health programs. Discounts offered are not offered as part of any advertisement or solicitation, and are based on the determinations of financial hardship in accordance with this policy.

3.  Private Insurance Contracts.  Discounts offered to privately insured patients are subject to limitations set forth by private insurance contracts.  In somes cases, contracts between our practice and insurance plans may require that we bill the insurer and charge patients for the applicable deductible and copayment amounts set forth by the contract.  Madras Physical Therapy reserves the right to review private insurance contracts and deny patient discounts as applicable.

4. Documentation of Payment. Under all circumstances, Madras Physical Therapy provides each patient with accurate statements of Madras Physical Therapy usual and customary charges for services, any patient discounts applied, and the amount pain(???) by the patient for services rendered.  If submitting these statements to insurance plans, patients are expected to report the actual amount they paid for services.
